Reporting Changes in Your Practice
Important changes to report to ISMIE include:
- New procedures performed.
- Acceptance of a medical director position.
- Changes in practice location or mailing address.
- Changes in partners, shareholders, directors or other professional and allied health professional personnel.
It is a good habit to immediately notify ISMIE’s underwriting division of changes to ensure adequate coverage.
| ISMIE policyholders can report changes by downloading a Change Request Form to your computer, then forward the completed and signed form to ISMIE via fax at 312-782-2023. The underwriter assigned to service your account will immediately process your request and confirm your changes in writing.
