ISMIE Mutual Insurance Company, its affiliates and subsidiaries ("ISMIE"), is committed to protecting and respecting your privacy. This policy describes how ISMIE handles the information obtained from our websites, portals, and our products and services that collect data. ISMIE maintains processes and procedures, both physical and electronic, in order to safeguard your information and to comply with federal and state laws.  Information we obtain from you, and information you voluntarily provide to us through ISMIE websites, may be used to provide you with information we believe may be of interest to you. We may also share the information with third parties who provide services to us. We retain your information securely in our database and will only keep your information for a reasonable period of time, or until you provide us notice of your desire for it to be removed or limited.

INFORMATION YOU PROVIDE TO US

We do not collect personally identifiable information unless you voluntarily provide it to us when utilizing a service, requesting information or logging onto one of our application systems. We do not require you to provide this information in order to view our sites or have access to most of the site content.
We do not disclose personally identifiable information collected through our site unless we have your consent, or unless disclosure is otherwise permitted or required by law. Credit card transactions enabled by our website are processed through a PCI DSS certified credit card processing company. We do not collect or retain credit card information through our website. WHAT ARE YOUR RIGHTS

At your request, ISMIE will remove from its files any contact information you provided. If you use our website to reach any of our specific software applications, additional policies may apply. You have certain rights over your personal information. The rights you will be able to exercise will depend on how and why ISMIE uses your information. ISMIE will not discriminate against you for exercising your right to request that your personal information not be collected, sold or disclosed.

You, or your authorized agent, have the right to know what Personal Information has been collected, disclosed, or used about you in the previous 12 months. This includes the right to:

  • request the categories of personal information sold, and the categories of third parties to whom is was sold to;
  • request the categories of personal information disclosed;
  • request the categories of sources where personal information was collected;
  • request the business purpose for which personal information was collected;

  • request your personal information not be sold.

HOW TO MAKE A REQUEST

To request that your personal information be limited, deleted, or to request additional records about the management of your personal information, please email your request to info@ismie.com or call 312-853-2055 or 800-782-4767 ext #2055.

  • Once we receive your request, we will evaluate and process it subject to exceptions as outlined by law.
  • Once the request is verified, we will then attempt to provide a response to your request within 45 days from the time it was received. We will notify you if we need additional time to respond to your request (up to an additional 45 days).

PROTECTION OF PERSONAL INFORMATION

ISMIE has implemented appropriate physical, electronic, and administrative procedures to protect the personally identifiable information we collect online. The forms used on our website to collect personally identifiable information are secure forms and are indicated by the letters "HTTPS" in the URL of the form.

However, when communicating via email, ISMIE is not responsible for the privacy of the messages in either direction. ISMIE cannot guarantee the security of any information you choose to disclose in an email. If you wish to communicate with ISMIE via a secure email system, we can provide you with login information so that the communications can be secure.

PROTECTED HEALTH INFORMATION (PHI)

ISMIE uses appropriate safeguards to prevent unauthorized use or disclosure of Protected Health Information (PHI) other than as permitted or as required by law. ISMIE utilizes administrative, physical, and technical safeguards that reasonably and appropriately protect the confidentiality, integrity, and availability of PHI that it receives.

CHANGES TO THIS POLICY

If this privacy policy changes, we will post the changes at https://www.ismie.com/privacypolicy with a link to the revised policy on the homepage of this site. You can tell when this policy was last updated by looking at the date at the end of the policy. Any changes will become effective upon posting of the revised policy as described. CME PRIVACY

We respect your privacy, and take great care to safeguard your Continuing Medical Education (CME) information in our possession. The protection and proper use of your personal information is of upmost importance.

Policyholder information (including email addresses) is not shared outside of our direct CME affiliates and partners. Any third-party companies used to provide you with CME products or services are required to keep your information secure and confidential.
