How to Talk with Patients About Advance Care Planning

National Healthcare Decision Day is Friday, April 16. It is a day set aside to encourage people to make their wishes known to their physicians about the medical care they’d like to receive if they are unable to speak for themselves because of an injury or illness.

Discussing wishes related to illness, death and dying can be challenging for patients, caregivers and physicians alike.
