New ISMIE resource provides guidance around patient discharge
Maintaining thoughtful and well-organized patient discharge processes is a critical safety component when coordinating patient care. Because of the high complexity involved in patient discharge and the need for collaboration among many different healthcare professionals, this process can be difficult for facilities to “get right.”
ISMIE’s new resource on patient discharge emphasizes that having strong discharge-related processes in place, as well as making sure all staff have the necessary relevant training, is of the utmost importance.
A seamless discharge process helps to ensure better patient outcomes and decreases the likelihood that a patient will have an unplanned visit back to the hospital.
